No one knows for sure past what is currently posted at each cruise line's health statements. Currently Carnival requires all passenger to be tested prior to boarding and provide a copy of the test results showing negative. All being boosted does for you is grant 1 extra day prior to boarding to do the test (again current protocols). Also since you are going to Italy as your final destination there maybe extra layers of testing or specific tests types that might have to be used. Unfortunately the knowledge on those won't happen until closer to your sail date.

Also just like with island ports closing quickly if there are local spikes in Covid numbers, the same could happen on a transatlantic. So make sure any return travel is flexible in nature and that you have read and understand exactly what is covered by your travel/vacation insurance.
