  1. Recently completed a southern caribbean cruise which stopped in several non-US territories, as well as a few US (Puerto Rico, USVI, etc.). Covid testing is normally required for foreign travelers to the US. However in our case, there was no required testing for return to the US after the international travel. At every port, some passengers and crew mingle with the locals, and performers are picked up or dropped off along the way, so there's plenty of opportunity to acquire covid during the journey. Then at the end of the cruise, crowds of passengers with no social distancing, stand in line to go through customs. The cruise itself had relatively good precautions, but we saw some missteps, such as packed tour busses (on ship's excursions), shared pens at customer service desk, and a minority of passengers flouting mask rules (wearing them on their chins, or not at all).
  2. You can buy travel insurance, and check to confirm that it covers this scenario. Royal Caribbean was providing transport home, but I'm not sure if that's still the case nor if it covered international transport. I was on Explorer of the Seas last week and a few crew members tested positive. There was no on-board testing of passengers (unless perhaps those with symptoms presented themselves to the medical clinic onboard). So, unless you display symptoms, its unlikely that you will be tested. We brought our own self-test kits with us as a precaution. Our ship wasn't much over 50% capacity, so there were few crowded areas and less potential for exposure. I'd also suggest avoiding potentially crowed/enclosed areas. The worst we experienced were an excursion bus (every seat taken), and the disembarkation line of people waiting to go through customs.
